How the Cumberland County Medication Access Program works for you and your patients
  1. The physician identifies a patient who is stabilized on a medication for a chronic condition and has problems paying for the medicine.
  2. The physician writes a prescription for a patient.
  3. The physician instructs the patient to mail in an application.
  4. The CCMAP Coordinator will screen the patient for financial eligibility and the availability of a program to provide the needed medication or the equivalent.
  5. CCMAP will re-write the prescription on a CCMAP form. In the event that the medication is not available through an assistance program but an equivalent is, CCMAP will contact the physician for a recommendation about substitution.
  6. Prescriptions written on CCMAP forms and pharmaceutical
    company paperwork will be sent to the physician for signatures.
  7. After signing, the physician will return the forms to CCMAP by mail or by delivery to the Pharmacy Department at Cape Fear Valley Medical Center.
  8. The medication will be shipped to CCMAP (average of 4-6 weeks)
    and dispensed from the CCMAP Pharmacy.
  9. The physician's office will be notified when renewal of the prescription is needed.
CCMAP will dispense samples of medications as available.
